Job summary

Sunnyvale Post Acute in Sunnyvale, CA is hiring a Speech-Language Pathologist (SLP) to join our rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunities to help residents improve communication, swallowing, and quality of life.

You will evaluate and treat residents with speech, language, cognitive, and swallowing needs, collaborate with an interdisciplinary team, and document care plans. California SLP license required; new grads welcome.

Qualifications

• Clear and effective communication with residents, families and staff.
• Ability to work within an interdisciplinary rehabilitation team.
• Strong organizational and documentation skills.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.
• Provide skilled therapy services based on resident needs and established care plans.
• Monitor resident progress and adapt treatment approaches as needed.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, therapy, dietary, and other team members.
• Educate residents, families, and caregivers on treatment recommendations.
• Promote safety, independence and quality of life for residents.
• Maintain compliance with professional and state/federal standards.
