Job summary

Sunnyvale Post Acute in Sunnyvale, CA is hiring a Speech-Language Pathologist to join our rehabilitation team. We offer full-time, part-time, and PRN opportunities to help residents improve communication, swallowing, and quality of life.

You will evaluate and treat residents with speech, language, cognitive, and swallowing needs, collaborate with nursing and therapy staff, and document progress in a timely, compliant manner.

Qualifications

• Must hold a valid California SLP license.
• Certification/credentials required to practice in CA.
• Experience in skilled nursing, post-acute care, or rehabilitation preferred.
• Strong communication, clinical, and organizational skills.
• Compassionate, resident-centered approach.
• Ability to work effectively in an interdisciplinary team.
• New graduates encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.
• Provide skilled therapy services based on resident needs and established care plans.
• Monitor resident progress and modify treatment approaches as appropriate.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, therapy, dietary, and other interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment recommendations and strategies.
• Promote resident safety, independence, communication, and quality of life.
• Maintain compliance with applicable professional, state, and federal standards.
