Job summary

Sunnyvale Gardens Post Acute in the United States is seeking a Speech Language Pathologist (SLP). We offer full-time, part-time, or PRN opportunities within an in-house therapy program. SLP CCC only; CFY not accepted. Competitive pay and comprehensive benefits are available.

Responsibilities include developing treatment plans, treating patients per physician plans, and coordinating with the health team. Involvement in conferences, documentation, discharge planning, and staff training is expected.

Qualifications

• Bachelor's degree in Speech-Language Pathology is required.
• Masters or Doctorate in Speech-Language Therapy preferred.
• CCC certification required; CFY not accepted.

Responsibilities

• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Communicate with supervisor and health team about patient progress and plans.
• Participate in Patient Care Conferences and Rehabilitation Conferences as needed.
• In-service training for other staff in the facility.
• Record evaluations and treatment notes per state laws and payer requirements.
• Record treatment changes per policy and procedures.
• Instruct families or nursing staff in maintenance and discharge preparation.
• Participate in discharge planning and act as clinical preceptor for CFY/RFP SLPs.
• Comply with State Practice Act and facility policies.

Education

Bachelor's degree in Speech-Language Pathology

Masters or Doctorate preferred

CCC certification required
