Job summary

Concho Health and Rehabilitation Center in Texas seeks a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will help residents improve independence and safety through meaningful daily activities and functional outcomes.

The position requires an active Texas OT license; SNF or long-term care experience is preferred. Familiarity with Net Health Electronic Documentation System is a plus, with flexible schedules offered to support patient-centered care.

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize ADLs and IADLs.
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities.
• Recommend adaptive equipment and provide training to support independence and safety.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team.
• Provide clinical education and support to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Comply with all applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab
