Job summary

Southside Behavioral Health provides case management services to high-risk children (ages 3-18) enrolled in the Mecklenburg County school system. The role includes managing caseloads, coordinating with schools, and delivering crisis interventions to support families.

The position is full-time and requires a Bachelor's degree in a human services field; QMHP-C is preferred and must be obtainable. A valid driver's license and background checks are required.

Qualifications

• Bachelor's degree in human services, social work, psychology or any human services related field required.
• QMHP-C preferred, ability to obtain QMHP-C required. Experience coordinating services to the mentally disabled/children at a local community level.
• Must meet all criminal background and drug screening requirements.
• Must be able to work independently.
• Valid driver's license and safe driving record required.

Responsibilities

• Prepare Individual Service Plans, Quarterlies, Whodas and other paperwork as required by Licensing & Medicaid.
• Carries a caseload of 35 to 40.
• Provide case management services to Seriously Emotionally Disturbed youth and families.
• Complete monthly home, school and community visits to families.
• Provide Juvenile Restoration Services to families.
• Coordinate services with other service providers in the community.
• Attend IEP meetings, coordinate with school staff as needed.
• Advocate for families and link to resources in the community.
• Provide crisis intervention and assist in crisis stabilization in home/school settings.
