Job summary

Primary Health Solutions is seeking a full-time Case Manager (School Based) to provide community psychiatric support to youth and families. You will develop treatment plans, work in the field, and coordinate with a multidisciplinary team to ensure client-centered care.

The role requires an Associate’s or Bachelor’s in human services, driving capability, and commitment to recovery-oriented approaches. You will document care and participate in supervision and case reviews as part of a dynamic team.

Qualifications

• Education/Experience: An Associate’s or Bachelor’s Degree in the human services field.
• Experience in providing services to families and children is preferred.
• Access to an operational vehicle and a valid driver’s license is required.
• Auto insurance with 100,000/300,000 liability policy coverage must be maintained.

Responsibilities

• Establish rapport with youth and families and assess treatment needs to reach goals.
• Provide mental health interventions in person, aiming for monthly contact.
• Coordinate ongoing assessment and monitoring of symptoms and functioning.
• Deliver crisis intervention to clients in acute distress.
• Educate families on accessing community resources and link to services.
• Maintain documentation and progress notes per program standards.
• Collaborate with other providers to ensure comprehensive care.
