Job summary

Home Care RN Inc. in Colorado seeks a Clinical Support RN to back our field team with dedicated time for documentation and clinical support in the home.

The role focuses on maintaining high-quality care while preventing burnout by freeing nurses to stay current on charting. You will perform 1:1 in-home visits, complete OASIS assessments in Axxess EMR, and work with therapists and case managers to deliver coordinated care.

Qualifications

• Must hold an active Colorado RN license.
• Solid knowledge of home health care practices and regulations.
• Experience with case management is preferred.
• Basic Life Support (BLS) certification required.
• Ability to work independently and manage time efficiently.

Responsibilities

• Provide 1:1 in-home skilled nursing visits for patients recovering from illness, surgery, or managing chronic conditions
• Complete thorough OASIS assessments and documentation via Axxess EMR
• Collaborate with therapists, aides, and case managers to deliver team-based care
• Educate patients and families to promote safety, independence, and health outcomes
• Maintain compliance with regulatory and professional standards of practice
