Qualifications

• Active RN license.
• Graduate of an accredited nursing program.
• Current CPR certification.
• Strong clinical assessment, communication, and organizational skills.

Responsibilities

• Provide skilled nursing care according to physician orders and patient care plans.
• Conduct patient assessments and ongoing evaluations.
• Coordinate care and communicate patient status changes with physicians and care team members.
• Educate patients and families on disease management, medications, and home care needs.
• Complete accurate and timely clinical documentation, including OASIS assessments.
• Participate in care planning, case conferences, and quality improvement activities.
• Supervise LPNs, CNAs, and Home Health Aides in accordance with regulatory requirements.
• Support patients' preventive, rehabilitative, and ongoing healthcare needs.
