UnityPoint Health is seeking an experienced RN Case Manager for UnityPoint at Home in Cedar Rapids to deliver in-home patient care with full clinical autonomy from admission to discharge. Your focus is on high-quality outcomes and patient comfort in the home setting.

This role requires an active RN license, 1+ year of similar nursing experience, and a valid driver's license. Schedule is 8:00am-4:30pm with some call and rotating weekends/holidays, plus a $10,000 sign-on bonus and benefits.

Qualifications

• Must have valid driver's license and auto insurance.
