Job summary

BrightSpring Health Services is seeking an experienced physical therapist to deliver functional rehabilitation in home, school, work, or community settings in California.

You will evaluate, create and adjust treatment plans, and ensure safe discharge and continuity of care while coordinating with families and the treatment team. A California license, CPR/BLS, and up-to-date credentials are required.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted license as a Physical Therapist in the state of practice.
• Current Basic Life Support or CPR certification as required.
• Minimum of one year of work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to neurological injury.
• Communicates effectively in verbal and written interactions.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans and participate in staffing meetings and family conferences.
• Perform discharge evaluations and provide home and community safety recommendations.
• Communicate patient needs and progress to the treatment team, physician, client, and family.
• Collaborate with other disciplines to coordinate care and improve patient outcomes.
