Job summary

BrightSpring Health Services, through Rehab Without Walls Neuro Rehabilitation, seeks a licensed Physical Therapist to support home and community-based rehab in Santa Cruz. The PRN role emphasizes functional rehabilitation across real-life settings, with autonomy and strong team collaboration.

Responsibilities include evaluations, treatment planning, regular reassessment, discharge planning, and inter-disciplinary communication.

Qualifications

• Must hold a current, unrestricted PT license in the practicing state
• Experience with functional rehabilitation in home/community settings is preferred
• Ability to lift 50 pounds and assist with patient transfers

Responsibilities

• Perform evaluations and develop treatment plans
• Regularly re-assess effectiveness of treatment plans and participate in staff meetings
• Perform discharge evaluations and provide home/community safety recommendations
• Communicate patient needs and progress to the treatment team and family
• Collaborate with other disciplines to coordinate care
