Job summary

OneWell Health Care is seeking a Per Diem Occupational Therapist (OTR/L) to provide evaluation, treatment, and support services to individuals with IDD in home and community settings in Paoli, PA. This field-based, per diem role offers flexible scheduling and per-visit compensation, with travel to client locations required.

The ideal candidate holds a current OT license (OTR/L), NBCOT certification, and experience delivering ADL-focused interventions, documentation in EHRs, and collaboration

Responsibilities

• Conduct functional, standardized, and observational assessments
• Develop and implement individualized treatment plans to improve ADLs
• Provide one-on-one therapy in home and community settings
• Monitor progress and adjust interventions as needed
• Recommend assistive devices and environmental modifications
• Collaborate with interdisciplinary teams and caregivers
• Educate and train families and support staff
• Maintain compliance with regulatory and documentation standards
• Participate in care coordination and support program improvement initiatives
