Palomacares in Tyler, TX is seeking an experienced Home Infusion Registered Nurse to provide skilled infusion therapy in patients' homes. The RN will travel locally to administer IV medications, biologics, IVIG, antibiotics, and hydration in accordance with physician orders and agency policies.

The ideal candidate holds a Texas RN license, current BLS/CPR, at least 1 year of RN experience, and strong assessment, documentation, and patient education skills.

Formación

• Current, unrestricted Texas RN license.
• Current BLS/CPR certification.
• Minimum of 1 year RN experience.
• Competency in peripheral IV insertion and central-line care.
• Strong patient assessment, documentation, and education skills.
• Ability to work independently in patients' homes.
• Reliable transportation and driver's license.
• Smartphone and internet access for scheduling and documentation.
• Ability to meet background and health-screening requirements.
• Willingness to travel within the assigned territory.

Responsabilidades

• Provide home infusion therapy, including IV medications, biologics, IVIG, antibiotics, hydration, and other prescribed therapies.
• Perform comprehensive patient assessments before, during, and after each infusion.
• Initiate and manage peripheral IV access and care for PICC lines, ports, and other vascular access devices.
• Monitor patients for adverse reactions and respond appropriately.
• Educate patients and caregivers on medications and infusion procedures.
• Communicate changes in patient condition to the pharmacy, physician, case manager, and clinical team.
• Complete accurate and timely electronic clinical documentation after each visit.
• Maintain infection-control and medication-safety standards in the home environment.
• Travel throughout the Tyler service area for scheduled visits.
• Track supplies used and promptly report any supply or medication concerns.
