Palomacares is seeking an experienced Home Infusion Registered Nurse in Abilene, Texas. The RN will travel to patients' homes to deliver infusion therapy and ensure care aligns with physician orders and safety standards.

This part-time/PRN role offers flexible scheduling, local travel within the Abilene service area, and the opportunity to provide high‑quality infusion care in patients' homes.

Qualifications

• Current, unrestricted Texas RN license.
• Current BLS/CPR certification.
• Minimum of 1 year of recent RN experience.
• Competency in peripheral IV insertion and central-line care.
• Ability to work independently in patients' homes.
• Reliable transportation, valid driver\'s license, and willingness to travel within the assigned territory.
• Smartphone and reliable internet access for scheduling and electronic documentation.
• Ability to meet applicable background, health-screening, and employment-compliance requirements.

Responsibilities

• Provide home infusion therapy, including IV medications, biologics, IVIG, antibiotics, hydration, and other prescribed specialty therapies.
• Perform comprehensive patient assessments before, during, and after each infusion.
• Initiate and manage peripheral IV access and provide care for PICC lines, ports, and other vascular access devices within scope and competency.
• Monitor patients for adverse reactions and respond appropriately.
• Educate patients and caregivers on medications, infusion procedures, vascular access care, and signs or symptoms requiring medical attention.
• Communicate changes in patient condition to the pharmacy, physician, case manager, and clinical team as appropriate.
• Complete accurate and timely electronic clinical documentation following each visit.
• Maintain infection-control and medication-safety standards in the home environment.
• Track supplies used and promptly report any supply or medication concerns.
• Travel throughout the Abilene service area for scheduled patient visits.
