Job summary

Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) to provide 1:1 evaluations and therapy in patients' homes across Orlando, FL. The role offers a flexible schedule, autonomy, and earnings of $75+ per visit with Medicare differentials.

You will assess speech, language, cognitive, and swallowing function, develop treatment plans, and collaborate with physicians and care teams while maintaining work-life balance with no required weekends or hospital shifts.

Qualifications

• Active Florida Speech-Language Pathologist license.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
