Job summary

Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) to provide 1:1 in-home evaluation and therapy in the Tampa area, covering The Villages and Lake County. Enjoy autonomy, a flexible schedule, and the ability to control income with compensation of $75+ per visit plus Medicare differential payments.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver’s license with reliable transportation and auto insurance.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
