Job summary

Concierge Home Care in Crestview, FL is seeking a Home Health Speech-Language Pathologist (SLP) to provide 1:1 patient care in the home with a flexible schedule and autonomy. The role offers earnings of $70+ per visit with additional Medicare differential compensation, covering Crestview and surrounding areas.

The ideal candidate will conduct comprehensive evaluations, develop individualized plans, and collaborate with physicians and the interdisciplinary team.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license
• Valid driver’s license and reliable transportation
• Home health experience preferred but not required
• OASIS/Medicare documentation experience preferred

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy for communication, cognition, and dysphagia
• Educate patients and caregivers to improve functional outcomes
• Collaborate with physicians and interdisciplinary teams
• Complete OASIS documentation as required
• Document visits using streamlined, AI-assisted technology
