Qualifications

• 2-4 years of experience in case management and/or behavioral health setting preferred.
• 1-2 years of experience leading teams preferred.
• Valid driver's license and reliable transportation required.

Responsibilities

• Provide wraparound services to families with a limit of up to 15 members.
• Coordinate comprehensive support for families.
• Conduct home visits with youth and/or family.
