Job summary

Professional Case Management (PCM) is seeking RN Case Managers to provide in-home nursing visits for former nuclear workers with chronic or terminal illness in New Mexico. You will work on your own schedule, with visits typically 1–2 hours weekly, perform vitals and safety checks, and oversee caregivers to ensure quality home care.

Applicants should be a graduate of an approved nursing program with an unrestricted RN license; BSN preferred. Supervisory or case management experience is a plus.

Qualifications

• RN license in state(s) of practice
• CPR certification maintained
• Minimum 2 years nursing experience, incl. 1 year in home care
• 1 year supervisory/case management experience preferred
• BSN preferred if possible

Responsibilities

• Conduct in-home nursing visits for assigned clients (weekly, 1–2 hours)
• Perform vitals and health assessments during visits
• Ensure home safety with medical equipment and supplies
• Oversee caregivers providing in-home care
• Plan, implement and evaluate client care; coordinate with regional director
• Participate in client care conferences and staffing decisions
• Maintain accurate documentation and compliance with policies
