Professional Case Management in New Mexico is seeking RN-Case Managers to perform in-home nursing visits for former Nuclear Weapons Workers with chronic and terminal illnesses. You will assess health needs, coordinate care, and ensure safety in the home, while managing patient care plans with weekly visits.

Qualifications

• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• BSN preferred.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided; Participate, coordinate and manage client care conferences as needed.
• Perform supervisory visits and assist staffing coordination as needed.
• Ensure adherence to Agency policies and nursing delegation guidelines.
