Job summary

Professional Case Management is hiring RN-Case Managers in New Mexico to conduct in-home nursing visits for former Nuclear Weapons Workers with chronic/terminal illnesses. You will provide direct care, perform vitals and assessments, and coordinate care in clients' homes on a weekly basis.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two years nursing experience including one year in home care or closely related field.
• One year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services.
• Plan, implement, and evaluate care provided.
• Coordinate and manage client care conferences as needed.
• Serve as a local on-site clinical resource and provide support to ensure home care needs are met.
• Assist with staffing coordination and scheduling within scope of practice and client needs.
• Adhere to nursing delegation guidelines and agency policies.
• Participate in interviewing, selection, and ongoing evaluation of clinical personnel.
• Review and evaluate clinical documentation for accuracy and completeness.
• Participate in performance improvement initiatives including quarterly medical record review.
