Job summary

Dimensional Health Care Associates, Inc. (DHCA) is seeking a Registered Nurse Case Manager/Delegating Nurse (RN CM/DN) to provide ongoing care for clients in the Baltimore area, with flexible scheduling and regional travel as needed. This role includes visiting clients, establishing nursing plans, delegating tasks, training staff, and ensuring compliance with state policies. No on-call is required and a competitive benefits package is offered.

Qualifications

• Must have Maryland RN license (compact) to practice as an RN in the setting.
• No prior experience required; training provided on the job.
• Experience using Google Platform and navigating software programs.

Responsibilities

• Provide ongoing care to assigned clients according to DHCA policies and guidelines.
• Travel to client locations for delegation of and oversight of delegable tasks to staff.
• Visit clients every 45 days for reassessment and care planning.
• Conduct Interim visits within 48 hours to address urgent needs and observations.
• Delegate administration of medications to certified Medication Technicians and train staff on nursing plans.
