Job summary

Healthcare Services Group Inc. is seeking a Registered Dietitian to manage clinical nutrition services at a long‑term care facility.

This role involves assessing resident needs, creating and documenting care plans, and providing nutrition education to residents, families, and staff. You will collaborate with interdisciplinary teams to improve health outcomes, ensure CMS and state compliance, and possibly cover multiple buildings as needed.

Qualifications

• Registered by the Commission on Dietetic Registration.
• State license in good standing.
• Knowledge of state survey process and nutrition care regulations (preferred).
• Strong organizational and communication skills.
• Positive attitude and effective communication with staff and leadership.
• COVID-19 vaccination policy compliance as required.
• If multi-site RD, must have a valid Driver's License.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and document the plan in the EMR.
• Develop, implement, and evaluate individualized care plans based on clinical findings, labs, weight trends, intake, and comorbidities.
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to determine a care plan improving health and well-being.
• Ensure adherence to CMS, state, federal regulations and company standards.
• Provide coverage across multiple buildings based on company needs.
• All other duties as assigned.
