Job summary

Aveanna Healthcare in Wisconsin is seeking a PRN Physical Therapy Assistant to assess, teach, and deliver therapy interventions under the guidance of the physician and physical therapist in the home health setting.

The role emphasizes one-on-one patient care in the Fox Valley area, with 1-3 days per week and no weekend work, allowing flexibility while helping patients maintain independence at home.

Qualifications

• Graduation from a Physical Therapist Assistant education program from an accredited university/program.
• Current, unrestricted state license as a Physical Therapy Assistant in the state of practice
• Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures

Responsibilities

• Provides PT sessions per the physician's plan of care and guidance from the physical therapist.
• Communicates with the patient and caregiver about the plan of care and progress.
• Delivers skilled therapy services and treatments appropriate for patient safety and function.
• Reports patient condition changes to supervisor and physician as needed.
• Educates patients and caregivers on safe use of prosthetic/orthotic devices.
