Total Care Therapy is seeking a compassionate Physical Therapist to join our expanding team in Grand Rapids. You will evaluate residents, develop individualized plans, and implement treatments to improve mobility, reduce pain, and promote independence within assisted and independent living communities.

Qualifications

• Current State Physical Therapist (PT) license.
• Current CPR/BLS certification.
• Valid driver's license with reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
