Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one geriatric PT in homes and senior living communities in Michigan. You will evaluate patients, craft individualized plans, and deliver evidence-based therapy to improve mobility and independence.

Clinicians collaborate with PTs, OTs, SLPs, and Fitness Specialists, with optional telehealth and flexible scheduling. Work in a supportive, clinician-driven culture that values high-impact patient outcomes.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.

Education

Degree from an accredited Physical Therapy program

Physical Therapy License in the state(s) of practice
