Job summary

PurposeCare is a leading home health provider, serving the Midwest, seeking a dedicated Physical Therapy Assistant to help patients regain mobility in their own homes. You will join a growing team and contribute to high-quality, compassionate care that keeps clients independent.

You will work under licensed PTs, perform skilled visits, follow care plans, and support safe, effective therapy. Requirements include licensure, 1 year of home health experience preferred, ability to lift up to 50 lbs,

Qualifications

• Current Physical Therapist Assistant license in the state of applying.
• 1 year of Physical Therapy Assistant experience preferred, home health experience desired.
• Ability to perform physically demanding tasks, including lifting up to 50 lbs, bending and standing.
• Valid Driver's License, vehicle insurance, and/or reliable transportation; ability to pass background check.

Responsibilities

• Provide skilled Physical Therapy visits to patients in their homes as prescribed.
• Follow treatment plans and document patient progress for supervising PTs.
• Support patient safety during therapy and coordinate with the care team.
