Job summary

Chandler Creek Post Acute in Greer, SC is seeking a Speech-Language Pathologist (SLP) to join our rehab team on a PRN basis.

Evaluate and treat residents with speech, language, cognitive, and swallowing disorders, collaborating with physicians, nurses, and therapists to create individual care plans. Flexible scheduling and opportunities across our SC network are available.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology.
• Active, unencumbered SLP license in South Carolina (required).
• CCC-SLP certificate preferred.
• Experience in a skilled nursing or rehab setting preferred.
• Strong knowledge of speech, language, cognitive, and swallowing disorders.
• Familiarity with PCC and Casamba preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Evaluate residents within 24 hours (or 1 business day) of physician referral.
• Develop and implement individualized treatment plans with physician approval.
• Provide therapeutic interventions based on patient needs and goals.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain accurate and timely clinical documentation.
• Participate in care conferences, discharge planning, and interdisciplinary meetings.
• Educate staff and caregivers on therapy techniques and maintenance programs.
• Supervise Speech-Language Pathologist Assistants (SLPAs) as applicable.
• Conduct in-service training for facility staff.
