Job summary

Chandler Creek Post Acute is seeking a dedicated Speech-Language Pathologist (SLP) to join our full-time rehab team in Greer, SC. You will help residents improve communication, cognition, and swallowing while collaborating with physicians, nursing, and therapy staff.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology.
• Active, unencumbered SLP license in South Carolina (required).
• CCC-SLP preferred.
• Experience in a skilled nursing or rehab setting preferred.
• Knowledge of speech, language, cognitive, and swallowing disorders.
• Experience with PCC and Casamba preferred.

Responsibilities

• Perform evaluations within 24 hours (or 1 business day) of physician referral.
• Develop and implement individualized treatment plans with physician approval.
• Provide therapy services in accordance with treatment plans.
• Monitor patient progress and adjust care as needed.
• Maintain accurate clinical documentation (evaluations, progress notes, discharge planning).
• Participate in care plan meetings, utilization review, and interdisciplinary conferences.
• Educate residents, families, and staff on therapy programs and techniques.
• Supervise SLPAs and CFYs as applicable.
• Conduct staff training and in-service education.
• Ensure compliance with all regulatory and facility standards.
