Job summary

Chandler Creek Post Acute in Greer, SC seeks a Speech-Language Pathologist (SLP) on a PRN basis to evaluate and treat residents with speech, language, cognitive, and swallowing disorders. You will collaborate with physicians, nursing, and therapy staff to develop individualized care plans and improve outcomes.

Qualifications

  • Master’s or Doctoral degree in Speech-Language Pathology.
  • Active, unencumbered South Carolina SLP license required.
  • CCC-SLP credential preferred.
  • Experience in a skilled nursing or rehab setting preferred.
  • Familiarity with PCC and Casamba preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Responsibilities

  • Evaluate residents within 24 hours (or 1 business day) of physician referral.
  • Develop and implement individualized treatment plans with physician approval.
  • Provide therapeutic interventions based on patient needs and goals.
  • Monitor patient progress and adjust treatment plans accordingly.
  • Maintain accurate and timely clinical documentation.
  • Participate in care conferences, discharge planning, and interdisciplinary meetings.
  • Educate staff and caregivers on therapy techniques and maintenance programs.
  • Supervise Speech-Language Pathologist Assistants (SLPAs) as applicable.
  • Conduct in-service training for facility staff.

We are seeking a skilled and motivated Speech-Language Pathologist (SLP) to join our rehab team on a PRN basis. Your expertise will play a key role in improving communication, cognition, and swallowing function for our residents.

Position Summary

As a Speech-Language Pathologist (SLP), you will evaluate and treat residents with speech, language, cognitive, and swallowing disorders. You will collaborate closely with physicians, nursing staff, and therapy team members to create individualized care plans and improve patient outcomes.

  • Strong knowledge of speech, language, cognitive, and swallowing disorders
  • Familiarity with PointClickCare (PCC) and Casamba preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
