Job summary

Edenbrook South is seeking a dedicated PRN Physical Therapist Assistant to develop and implement individualized therapy programs for residents, promoting independence and a healthy lifestyle under the supervision of a licensed PT. This role focuses on evidence-based rehabilitation within skilled nursing settings.

Qualifications

• Active Pennsylvania PTA license in good standing.
• Degree from an accredited PTA program.
• Strong communication, documentation, and patient-care skills.
• Experience in skilled nursing, home health, or rehab settings preferred.

Responsibilities

• Maintain rehabilitation standards under supervision of a PT.
• Provide quality therapy services and document care.
• Plan and coordinate discharge and treatment schedules.
• Communicate and collaborate with the care team.
• Assist with data collection on patient progress.
• Attend care plan meetings as requested.
• Supervise PTA students as appropriate.
