Job summary

Autumn Lake at Greenfield in Milwaukee, Wisconsin is seeking a PRN Physical Therapist Assistant (PTA) to join its rehabilitation team. You will provide hands-on care under a licensed Physical Therapist, helping residents improve mobility and independence.

You will implement treatment plans, assist with gait, transfers, balance, and mobility, document progress, and collaborate with a multidisciplinary team. A valid Wisconsin PTA license is required; strong communication and teamwork are

Qualifications

• Active Wisconsin PTA license required.
• Strong communication and teamwork skills.
• Skilled nursing or rehabilitation experience preferred.

Responsibilities

• Provide treatments under the direction of a licensed Physical Therapist.
• Assist residents with strength, balance, gait, transfers, and mobility.
• Carry out individualized treatment plans based on resident goals.
• Monitor progress and communicate updates to the supervising PT.
• Complete accurate and timely clinical documentation.
• Collaborate with the interdisciplinary care team.
