Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for hospice patients in a home setting in West Palm Beach, FL. This PRN role offers flexible assignments and a chance to support independence and quality of life for older adults.

Qualified candidates hold a Florida PT license with a PT degree (Bachelor, Master, or Doctorate) and are open to both new graduates and experienced therapists. Competitive pay and autonomy are highlighted in this role.

Qualifications

• Licensed Physical Therapist in Florida (PT) with an active license.
• Graduate degree in Physical Therapy (Bachelor’s, Master’s or Doctorate) from an accredited program.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
