Job summary

Sobe Innovative Rehab in Vero Beach, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in a home setting. This PRN role offers flexible assignments and the chance to improve quality of life for older adults.

You will evaluate and treat residents in home or assisted living settings, develop individualized plans, document in EMR, and collaborate with physicians and families. Florida licensure is required; new grads are encouraged to apply.

Qualifications

• Licensed Physical Therapist in Florida with an active license.
• Degree in Physical Therapy from an accredited program (Bachelors/Masters/Doctorate).

Responsibilities

• Evaluate and treat residents in an assisted living community or home setting.
• Develop and implement individualized treatment plans.
