Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy in home settings for residents of Vero Beach, FL.

This PRN role offers flexible assignments, clinical autonomy, and the opportunity to support independence and quality of life for older adults. Applicants must hold an active Florida PT license and a PT degree from an accredited program; new grads are encouraged to apply.

Qualifications

• Licensed Physical Therapist in Florida with an active license.
• PT degree from an accredited program (DPT/PT/RPT).
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.

Education

Bachelor's/Master's/Doctorate in Physical Therapy

Active Florida PT license
