Patewood Post Acute is seeking a compassionate Physical Therapist to join our rehabilitation team on a PRN basis in Greenville, SC. You will evaluate and treat residents, develop individualized plans, and work with an interdisciplinary team to restore mobility and independence.

The role emphasizes clinical excellence, clear documentation, and ongoing collaboration with physicians, nursing staff, residents, and families. We offer flexible PRN scheduling and competitive compensation.

Qualifications

• Bachelor’s degree in Physical Therapy required; Master’s or Doctorate preferred.
• SC license in good standing; continuing education requirements.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with Casamba and PointClickCare (PCC) preferred.
• Strong clinical, communication, and documentation skills.
• Must maintain continuing education requirements and stay in good standing with the State Board.

Responsabilités

• Evaluate residents and develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence.
• Monitor resident progress and modify treatment plans as appropriate.
• Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment goals and outcomes.
• Complete accurate and timely documentation in accordance with facility, state, federal, and payer requirements.
• Participate in care plan meetings, rehabilitation conferences, and discharge planning activities.
• Educate residents, families, and caregivers on therapy goals, safety techniques, and home exercise programs.
• Conduct in-service education and training for facility staff as needed.
• Ensure compliance with all state practice acts, facility policies, and regulatory requirements.
• Promote resident safety, independence, and quality of life.
