Job summary

Aventura at Assumption Village in North Lima, OH, seeks a Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. Full-time and PRN opportunities provide stable hours and flexible options while delivering hands-on therapy to residents to improve mobility and independence.

You'll work under a licensed Physical Therapist, implement treatment plans, document progress, and collaborate with nursing and the interdisciplinary team to support resident-centered care.

Qualifications

• Active Ohio PTA license in good standing.
• Experience in LTC/SNF or geriatric/post-acute care preferred but not required.
• Strong communication, organization and teamwork skills.

Responsibilities

• Provide skilled, hands-on therapy under a licensed PT.
• Implement individualized treatment plans.
• Address strength, mobility, balance, endurance and functional goals.
• Monitor resident progress and response to treatment.
• Educate residents and caregivers on exercises and mobility strategies.
• Document progress accurately and timely.
• Collaborate with nursing and interdisciplinary care team.
• Deliver compassionate, resident-centered care.
