Aventura at Walton Hills in Walton Hills, OH is seeking a PRN Occupational Therapist (OT) to join our in-house therapy team. This role offers flexible hours, competitive PRN compensation, and the chance to make a meaningful difference in residents’ lives.

Responsibilities include evaluating residents, developing individualized plans, addressing ADLs/IADLs, and collaborating with nursing and the care team. Ohio OT license required; experience in long-term care preferred but not required.

Qualifications

• Active Ohio OT license in good standing with licensing boards.
• Experience in long-term care or geriatrics preferred but not required.
• Strong communication and collaboration skills.

Responsabilités

• Evaluating residents and identifying functional needs.
• Developing and implementing individualized treatment plans.
• Addressing ADLs, IADLs and functional independence.
• Monitoring progress and adjusting treatment plans as needed.
• Recommending adaptive equipment and safety strategies.
• Educating residents, families, and caregivers.
• Collaborating with nursing and interdisciplinary care team.
• Completing timely clinical documentation.
• Participating in care planning and team collaboration.
