Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide Hospice therapy for residents in home settings in Dade City, Florida. The role is PRN with flexible assignments and no driving between homes, offering competitive compensation and the ability to choose your schedule.

You will evaluate and treat residents in home or assisted living environments, develop individualized treatment plans, collaborate with physicians, families, and the interdisciplinary care team,

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Active Florida Physical Therapist (PT) license
• New graduates and experienced therapists are encouraged to apply

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR

Education

Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
