Sobe Innovative Rehab in Lakeland, FL is seeking a licensed Physical Therapist (DPT, PT, RPT) to provide hospice therapy in a home setting on a PRN basis. You will help older adults maintain independence and quality of life through compassionate, evidence-based care.

Evaluate and treat residents in home or assisted living settings, develop individualized plans, and document in the EMR. Florida PT license required; new grads welcome. Flexible PRN schedule and competitive pay offered.

Qualifications

• Licensed Physical Therapist in Florida with an accredited degree.
• Florida PT license active and in good standing.
• New grads and experienced therapists encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
