Job summary

Sobe Innovative Rehab in Dade City, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in a home setting on a PRN basis. Join a compassionate team delivering evidence-based care to help older adults maintain independence and quality of life.

Qualifications

• Must have BA/MA/Doctorate in Physical Therapy from an accredited program.
• Active Florida PT license.
• New graduates and experienced therapists encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR

Education

Bachelor's/Master's/Doctorate in Physical Therapy
