Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in a home setting for Lake Alfred, FL. This PRN role offers flexible assignments and competitive pay with clinical autonomy within a supportive team.

You will evaluate and treat residents, develop individualized plans, document in the EMR, and collaborate with physicians and families. Florida PT license required; able to cover zip codes 33805 and 33801.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Active Florida Physical Therapist license.
• New graduates and experienced therapists are encouraged to apply.
• Able to cover patients in zip codes 33805 and 33801.

Responsibilities

• Evaluate and treat residents in a home setting or assisted living.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
