Job summary

PruittHealth @ Home is seeking a Physical Therapist to provide evaluative, consultative and treatment services in patients’ homes. The role emphasizes higher compensation tied to effort and time spent outside visits, with a patient‑centered approach and collaboration with a multidisciplinary team.

Responsibilities include assessing functional needs, developing care plans, documenting progress, and ensuring compliance with industry standards.

Qualifications

• Must have a valid professional license in the state of practice.
• Graduate of an accredited Physical Therapy Program.
• If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

Responsibilities

• Evaluate the patient’s level of function and skilled therapy needs.
• Treat the patient to relieve pain, develop or restore function, and obtain maximum outcomes according to the plan of care.
• Advise and consult the family and other interdisciplinary team members to promote functional outcomes.
• Document progress using required documents and timelines.
• Maintain continuity of care by collaborating with the interdisciplinary team and attending physician.
• Demonstrate knowledge of home care and competency for patient care skills.
• Conduct field visits to evaluate and initiate care and monitor outcomes.

Education

Bachelor’s degree in Physical Therapy

Foreign-trained evaluation when applicable
