Job summary

Avid Health at Home, LLC is seeking a PRN Physical Therapist to provide in-home physical therapy services under physician orders. The PT will evaluate patients, develop individualized treatment plans, implement therapeutic interventions, and collaborate with the interdisciplinary team to maximize patients' independence, safety, and quality of life.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current license to practice Physical Therapy in the state.
• CPR/BLS certification.
• Experience in home health preferred.
• Strong communication and organizational skills.

Responsibilities

• Perform comprehensive physical therapy evaluations to determine functional status and rehab needs.
• Develop individualized plans of care based on physician orders.
• Provide skilled PT interventions including therapeutic exercise and gait/balance training.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Document all evaluations and visits per agency policies and regulations.
• Collaborate with the interdisciplinary team to coordinate care.
