Concept Rehab in Lima, Ohio is seeking a PRN Physical Therapist to join our therapist-owned team. The role offers flexible scheduling, competitive compensation based on experience, and the opportunity to work with seasoned rehab professionals dedicated to patient-centered care.

As a PT, you will evaluate and treat patients with mobility and functional challenges, develop individualized plans, supervise assistants, and collaborate with physicians and staff to optimize outcomes.

Formación

• As a Physical Therapist, you will evaluate and treat patients in skilled settings.
• Current Ohio Physical Therapist license required.
• State and federal requirements must be met.
• Experience in skilled nursing/post-acute/geriatric settings preferred.
• Current CPR certification preferred.

Responsabilidades

• Evaluate patients for physical therapy services.
• Develop and implement individualized treatment plans.
• Provide skilled therapy interventions per care plans.
• Supervise Physical Therapist Assistants per state rules.
• Communicate with physicians, nursing, families, and team.
• Participate in care and discharge planning activities.
• Document evaluations, progress notes, and discharge summaries.
• Educate patients and staff on rehab programs.
• Conduct in-service education for facility staff.
• Maintain productivity and clinical quality standards.
• Promote Concept Rehab values: Commitment, Respect, Integrity.
