Concept Rehab, a therapist-owned rehabilitation company, seeks a PRN Physical Therapist in Lima, Ohio. You will evaluate and treat patients with mobility and functional challenges, developing individualized treatment plans and collaborating with a multidisciplinary team to deliver patient-centered care.

The role offers flexible PRN scheduling, opportunities for clinical growth, and a supportive leadership team committed to high-quality outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Ohio.
• Experience in skilled nursing, post-acute care, rehabilitation, or geriatric settings preferred but not required.
• Current CPR certification preferred.

Responsabilités

• Evaluate and treat patients with mobility, strength, balance, functional, and positioning challenges.
• Develop and implement individualized treatment plans.
• Provide skilled therapy interventions based on plans of care.
• Supervise Physical Therapist Assistants as allowed by state regulations.
• Communicate with physicians, nursing staff, families and interdisciplinary team members.
• Document evaluations, progress notes, and discharge summaries accurately and timely.
• Educate patients and families on restorative and maintenance programs.
• Promote concepts of commitment, respect, and integrity.
