Mission Valley Post Acute in Fremont, CA is seeking a compassionate Occupational Therapist for a flexible PRN on-call role. You will join our rehabilitation team to deliver high-quality, patient-centered care and support independence with tailored treatment plans.

Qualifications

• California OT license required.
• Strong clinical and communication skills.
• Post-acute or rehabilitation experience preferred.

Responsibilities

• Conduct occupational therapy evaluations and assessments.
• Develop and implement individualized treatment plans.
• Provide skilled OT interventions to improve independence and quality of life.
• Collaborate with interdisciplinary team to coordinate patient care.
• Supervise and guide COTAs.
• Educate patients and families on plans, discharge and home safety.
• Monitor progress and modify interventions as needed.
• Complete documentation accurately and timely.
