Job summary

Petaluma Post Acute Rehab in Petaluma, California is seeking a licensed Occupational Therapist for a PRN role. You will evaluate residents, develop and implement therapy plans, and collaborate with a rehab team in a 5-Star Medicare rated facility.

This position offers true scheduling flexibility, premium per-diem pay of $65–$70 per hour, and clinical autonomy to drive meaningful outcomes in ADLs, IADLs, and cognitive function within a caring team environment.

Qualifications

• Current, active, and unrestricted Occupational Therapist license in California.
• Graduate of an accredited Occupational Therapy program.
• NBCOT certification required.
• CPR/BLS certification or willingness to obtain.

Responsibilities

• Conduct comprehensive OT evaluations and re-evaluations consistent with physician orders.
• Develop and modify individualized plans of care with measurable goals.
• Deliver skilled OT interventions addressing ADLs, IADLs, cognition, and upper extremity rehab.
• Recommend and train on adaptive equipment and environmental modifications.
• Lead discharge planning and coordinate care transitions to home.
