Job summary

Mission Valley Post Acute in Fremont, CA is seeking a compassionate Occupational Therapist (OT) for a PRN role to support our rehabilitation team and provide patient-centered care.

You will perform comprehensive evaluations, develop and implement individualized treatment plans, supervise COTAs, educate patients and families, and collaborate with an interdisciplinary team to restore independence and safety.

A California OT license is required.

Qualifications

• Strong clinical and communication skills required.
• Ability to work effectively both independently and as part of a team.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and assessments.
• Develop and implement individualized treatment plans.
• Provide skilled occupational therapy interventions to improve functional independence.
• Supervise and guide Certified Occupational Therapy Assistants (COTAs).
• Educate patients and families regarding treatment plans and home safety.
• Maintain timely and accurate documentation.
