Job summary

BrightStar Care in Celina, Texas is seeking a PRN Occupational Therapist to provide one-on-one home health services. You will evaluate patients, develop and implement individualized treatment plans, and support independence, safety, and mobility in the patients' homes.

The role offers flexible scheduling, with visits accepted based on availability and patient needs. Travel to homes across the Celina area is expected, and prior home health experience is preferred but not required.

Qualifications

• Current, unrestricted Texas OT license.
• Strong communication and interpersonal skills.
• Ability to work independently and manage schedule.
• Proficiency in English.
• Reliable transportation and ability to travel to patients' homes.

Responsibilities

• Evaluate patients' functional abilities and needs related to daily living activities.
• Develop individualized occupational therapy treatment plans based on goals.
• Provide interventions to improve independence, safety, mobility and quality of life.
• Assist patients with improving ADLs and IADLs.
• Educate patients and caregivers on safety strategies and exercises.
• Monitor progress and adjust plans as needed.
• Document visits accurately per clinical and regulatory requirements.
• Communicate with physicians, caregivers, and the care team.
• Maintain compliance with healthcare regulations and standards.
• Provide compassionate, patient-centered care in home settings.
• Accept visits based on availability and patient needs.
